Student-Athlete Sports Physicals
As an athlete, keeping your edge is important to you. As your local health care provider, your health is important to us.
- Free yearly physical under the Health Care Reform Act with your primary care provider
- Sports physical fee of $30 payable at time of service
- Meets school and WIAA requirements
- Appointment dress code: shorts and gym shoes
- Bring completed WIAA sports card to appointment
- Parent/guardian must accompany athlete or sign the consent form below

DirectCare appointments are available daily from 7:30am to 4:30pm. $30 payment is due at time of service. Schedule online
