Student-Athlete Sports Physicals

• Free yearly physical under the Health Care Reform Act with your primary care provider
• Sports physical fee of $30 payable at time of service
• Meets school and WIAA requirements
• Appointment dress code: shorts and gym shoes
• Bring completed WIAA sports card to appointment
• Parent/guardian must accompany athlete or sign the consent form below

Appointments can be made by phone, through your MyChart account or in the MyChart app.
